Description

You will provide home health visits to adult and geriatric clients, performing diagnostic tests, developing and implementing individualized care plans, and educating clients and families on rehabilitative care. This role is part of a multi-disciplinary team focused on helping clients remain at home.

Requirements

• Current NJ Physical Therapist license
• Minimum one year recent work experience as a Physical Therapist
• Graduation from an APTA-approved program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Make home visits to clients in designated territories
• Perform diagnostic tests and measurements (mobility, strength, balance, etc.)
• Implement integumentary protective interventions for wound prevention and repair
• Develop and implement individualized care plans including manual exercises and gait training
• Document observations and interventions using a touch pad tablet
