Description

You will provide in-home physical therapy to adult and geriatric patients, helping them restore strength, balance, and mobility. You'll evaluate patients, develop treatment plans, and educate families on recovery and preventive care.

Requirements

• Active PT license in good standing
• BLS CPR certification
• Reliable transportation
• Basic comfort with technology and EMR systems

Responsibilities

• Deliver in-home PT services to adult and geriatric patients
• Evaluate mobility, strength, and function to develop treatment goals
• Implement rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery and preventive care
• Maintain accurate home health care documentation
