Description

As a Home Health Case Manager, you will conduct initial patient assessments, develop and revise care plans, administer medications, and coordinate with physicians and community resources to ensure high-quality, personalized care for elderly patients in their homes.

Requirements

• Graduate of approved nursing program
• Valid Ohio Registered Nurse (RN) license
• Current CPR certification
• Minimum 1-year clinical nursing experience
• Valid driver's license, reliable automobile, and clean driving record

Responsibilities

• Complete initial assessment of patient and family to determine home care needs
• Develop and revise plan of care based on nursing diagnosis and patient status changes
• Administer medications and treatments as prescribed by physician
• Counsel patient and family in meeting nursing and related needs
• Coordination of care plan with community health persons and physician
