Description

As a home care Medical Social Worker, you will be an integral member of a multi-disciplinary team providing rehabilitative care and skilled nursing to clients in their homes. You will conduct home visits, evaluate social and emotional factors, develop care plans, and connect clients with community resources to support their health and well-being.

Requirements

• Master of Social Work (MSW) with 2+ years community-based experience
• Licensed by NJ Board of Social Work Examiners (LMSW or LISW)
• Accreditation by Council on Social Work Education
• Ability to travel to cases
• Solid computer skills; EMR experience preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Assist in understanding social and emotional factors related to clients’ health problems
• Provide clinical social evaluations and plan appropriate interventions
• Identify community resource referrals for clients and families
• Participate in developing total plan of care with multidisciplinary team
