💼 About This Role

You'll provide home health visits to adult and geriatric patients, performing speech and swallowing evaluations as part of a multidisciplinary team. This role offers flexible part-time or per diem scheduling in Monmouth and Middlesex counties.

🎯 What You'll Do

• Conduct home visits for speech and swallowing evaluations
• Develop and revise treatment plans based on evaluations
• Educate patients and families on rehabilitative care
• Document care using a touch pad tablet

📋 Requirements

• Current Speech Language Pathologist license in New Jersey
• Certificate of Clinical Competence in Speech or equivalent
• Solid computer skills for electronic medical records

✨ Nice to Have

• Prior home care experience as an SLP
• Experience with electronic medical records (EMR)
