Description

You will organize, coordinate, and direct ongoing home health services for assigned patients, ensuring quality care and supervising LPNs and CNAs as needed. You'll perform assessments, develop care plans, document in the EMR, and participate in on-call and team meetings to promote positive patient outcomes.

Requirements

• Graduate of accredited school of nursing; BSN/MSN preferred
• Current RN Missouri license in good standing
• Minimum 2 years varied work experience as professional nurse
• 1 year in home health/hospice setting preferred
• Ability to lift/transfer patients weighing 10-200+ lbs and drive in all weather

Responsibilities

• Organize, coordinate, and direct ongoing provision of services to assigned patients
• Perform initial and follow-up comprehensive assessments and drug regimen reviews
• Develop and update plan of care with patient/family/physician/team
• Document in EMR at point of care and complete required physician summaries
• Participate in on-call schedule, case conferences, and QAPI program
