Description

As a Hospice RN Case Manager, you will provide compassionate end-of-life care to clients in their homes across Adams, Dauphin, Lancaster, and York counties. You'll administer medications, perform wound care, monitor conditions, and collaborate with the care team to ensure comfort and dignity for patients and their families.

Requirements

• Current RN license in Pennsylvania
• Minimum 1 year recent clinical (medical/surgical) experience
• Solid computer skills
• Ability to travel to cases
• Prior home care or hospice experience strongly preferred

Responsibilities

• Make home visits to clients in designated geographic territories
• Administer medication, wound care, treatments, and comfort care
• Monitor clients' conditions and report changes to managers
• Execute and document doctors' orders
• Perform client assessments as necessary
