Description

You will perform home health visits, providing medical nutrition therapy and dietetic education to clients and their families in Philadelphia, PA. You'll collaborate with a multi-disciplinary team to help clients remain safely at home.

Requirements

• Bachelor's degree in food and nutrition from an accredited institution
• Registered Dietitian license in Pennsylvania
• Minimum 2 years recent experience as a Registered Dietician in a healthcare setting
• Membership in the American Dietetic Association
• Solid computer skills; EMR experience preferred

Responsibilities

• Conduct home visits to clients in designated territories
• Plan, organize, and conduct dietetic education for staff, clients, and caregivers
• Assess, develop, and counsel on medical nutrition therapy for disease management and prevention
• Translate scientific findings into practical information for patients and families
• Document clinical treatment in electronic medical records
