You'll provide 1:1 home health visits to adult and geriatric patients in Lehigh County, following designated care plans. You'll make a direct impact on patient well-being while enjoying flexible scheduling and a supportive team environment.

🎯 What You'll Do

• Follow designated care plans per patient needs
• Administer medication, wound care, treatments, and procedures
• Monitor client conditions and report changes to manager
• Accurately document observations and interventions using a tablet

📋 Requirements

• Current Registered Nurse license in Pennsylvania
• Minimum one year of recent clinical (medical/surgical) experience
• Graduation from an accredited nursing program
