Description

You will lead a multi-disciplinary team of home health care professionals, directing and coordinating care, processing referrals, managing field staff, and ensuring clinical excellence and adherence to policies. This role combines internal case management with staff development and quality assurance in a Medicare-certified home health setting.

Requirements

• Active Hawaii Registered Nurse license
• Recent Medicare Certified Home Health Care experience
• Comprehensive knowledge of Medicare Conditions of Participation
• Understanding of OASIS documentation and ICD-10 coding
• Strong people management and leadership experience

Responsibilities

• Direct and coordinate care provided by multi-disciplinary home health team
• Process incoming referrals
• Manage and mentor clinical associates and field staff
• Facilitate new hire onboarding and orientation
• Review documentation including OASIS and HCHB; monitor quality assurance
