Description

You will provide skilled nursing care to home health patients under physician orders, supporting their families and caregivers. This role focuses on delivering compassionate, person-centered care as part of a faith-based ministry.

Requirements

• Graduate of approved nursing program
• Valid Ohio LPN license and CPR certification
• Minimum 2 years direct patient care experience in hospice or home care
• Valid driver's license, auto insurance, and clean driving record
• Ability to travel within service area (Lebanon, Middletown, Springboro, Dayton, Cincinnati)

Responsibilities

• Provide skilled professional nursing care to patients, families, and caregivers as prescribed by physician
• Comply with Home Health Medicare Conditions of Participation, Ohio licensure laws, and agency policies
• Travel locally to patient homes for care delivery
• Participate in on-call rotation
• Maintain patient documentation and coordinate care
