💼 About This Role

You'll be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to adult and geriatric patients in their homes. You'll perform evaluations and develop treatment plans to help clients remain at home.

🎯 What You'll Do

• Make home visits to clients in designated territories.
• Perform evaluations for speech, language, communication, or swallowing difficulties.
• Develop and revise treatment plans based on evaluations and physician orders.
• Educate patients and families on rehabilitative care and activities.

📋 Requirements

• Current SLP license in Pennsylvania.
• Certificate of clinical competence in speech from ASHA or equivalent.
• Solid computer skills; experience with EMR preferred.

• Prior home care experience as an SLP.
• Experience with electronic medical records (EMR).
