Description

You will provide rehabilitative care and skilled nursing to adult and geriatric clients in their homes, as an integral member of a multi-disciplinary team. Your work helps clients receive medical care while remaining at home, promoting their health, safety, and independent living.

Requirements

• Current North Carolina Physical Therapist license
• Minimum one year of recent work experience as a Physical Therapist
• Graduation from an APTA or CAAHEP accredited program
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills

Responsibilities

• Perform diagnostic tests and measurements such as mobility, balance, and strength assessments
• Implement integumentary protective interventions to prevent wounds and promote repair
• Develop and implement individualized care plans including manual therapeutic exercises and gait training
• Continually assess and revise care plans, participating in multidisciplinary planning
• Educate clients and family members on rehabilitative care and activities for independent living
