You'll lead a team of RN Case Managers and LVNs, ensuring coordinated patient care in a growing home health agency that's a 2024 Top Workplace. You'll conduct field supervisory visits and develop nursing education to reduce hospital readmissions below 10%.

🎯 What You'll Do

• Supervise RN Case Managers and LVNs in field patient care
• Handle incoming calls and follow up on patient care issues
• Coordinate training and educational programs for nursing staff
• Conduct bi-weekly case conferences to review patient progress

📋 Requirements

• Two years management or supervisory experience in healthcare
• CA Registered Nurse License (RN)
• Current Basic Life Support (BLS) Card
• Valid CA Driver's License with acceptable driving record
