💼 About This Role

You'll be Waymark's frontline presence in East Richmond, connecting Medicaid patients with social support and medical resources. You'll work on a multidisciplinary care team to improve health outcomes for low-income communities. This role offers a virtual 2-week paid training program and the chance to make a tangible impact on patient lives.

🎯 What You'll Do

• Conduct needs assessments and motivate patients to set health goals.
• Support patients with housing, behavioral health, and food insecurity.
• Create and maintain a directory of community resources.
• Document each patient encounter accurately and thoroughly.

📋 Requirements

• Highly organized and self-motivated to work independently.
• Cultural competency to work with diverse community members.
• Reside within a commutable distance of East Richmond, Virginia.
• Current driver's license and access to an insured vehicle.

• Experience conducting home visits and phone outreach.
