💼 About This Role

You'll provide hands-on patient care in home settings across Amador County with the largest home health agency in San Francisco. Your work will directly reduce hospital readmission rates to under 10%, far below the industry average.

🎯 What You'll Do

• Perform skilled nursing procedures like PICC line care and wound vac management.
• Implement and document care plans under a Case Manager's direction.
• Provide patient/family education on diagnosis and self-care techniques.
• Coordinate with interdisciplinary team (HHA, OT, PT, MSW, ST).

📋 Requirements

• Current CA Registered Nurse License (RN).
• One year of recent nursing experience.
• Current BLS certification.
• Valid CA Driver's License with acceptable record.
