Description

You will manage a diverse patient panel, conducting acute, chronic, and well visits in-office and via tele-health. Collaborate with your care team to deliver human-centered primary care and continuously learn through clinical rounds.

Requirements

• Completed accredited FNP or PA program with national certification
• Minimum 2 years outpatient primary care experience seeing all ages, or 1 year with primary care fellowship or 1+ year urgent care seeing all ages
• Licensed in Massachusetts before start date
• Passion for human-centered primary care and ability to communicate with diverse populations
• Clinical proficiency in evidence-based primary care and comfort using technology

Responsibilities

• Manage a patient panel with acute, chronic, and well visits
• Conduct in-office and tele-health visits
• Participate in weekly Clinical Rounds and ongoing learning
• Collaborate with in-office and virtual clinical teams via daily huddles
• Perform in-office procedures based on clinical training
