💼 About This Role

You'll join an interdisciplinary team to deliver restorative physical therapy services, focusing on maximizing participant independence and safety. Lower patient volumes allow you to build long-term relationships with participants, and you'll collaborate closely with other therapists and providers. This role offers work/life balance with no after-hours documentation.

🎯 What You'll Do

• Conduct physical therapy assessments in facility and occasionally at home
• Actively participate in IDT meetings to develop participant care plans
• Deliver and document interventions as agreed in care plans
• Provide active/passive exercise, gait training, and functional ADL training
• Supervise Therapy Assistants and implement Functional Maintenance Programs

📋 Requirements

• Bachelor's degree in Physical Therapy
• Two (2) years in clinical setting with frail or elderly population preferred
• Valid California Physical Therapy License or proof of eligibility
• Must obtain CPR and First Aid certification within 180 days of hire

• Master's or doctoral degree in physical therapy
• Outpatient, acute, inpatient, or skilled nursing experience
• CPR and First Aid certification upon hire
