You'll provide skilled home health nursing to seniors in the PACE program, coordinating care with an interdisciplinary team. Your work directly empowers participants to live healthier, more independent lives. This role stands out because you skip OASIS assessments and earn hourly pay that includes travel time.

🎯 What You'll Do

• Conduct home health nursing assessments and determine equipment needs.
• Participate in IDT meetings to develop participant care plans.
• Deliver nursing interventions, including medication administration and tests.
• Collaborate with physicians and IDT for smooth care transitions.

📋 Requirements

• Graduate of an accredited school of nursing
• Unencumbered RN License
• Minimum of 1 year nursing experience with frail or elderly population
• Reliable means of transportation
