Description

You will plan, coordinate, and document skilled home health nursing care, acting as a case manager to develop and implement individualized care plans in compliance with regulations. You'll travel within the greater Lebanon, Ohio area to provide care and supervise home health aides.

Requirements

• Valid Ohio RN license
• Current CPR certification
• Minimum 1 year clinical nursing experience
• Licensed driver with insured automobile
• Graduate of approved nursing program

Responsibilities

• Complete initial patient assessment and history
• Develop and revise plan of care based on patient needs
• Administer medications and treatments per physician orders
• Counsel patient and family on nursing needs
• Supervise and evaluate home health aide care
