💼 About This Role

You'll deliver comprehensive geriatric care and conduct Annual Wellness Visits and Advance Care Planning discussions for seniors in under-resourced communities. You'll help build and scale clinical programs at partner health centers, improving preventive care and health outcomes.

🎯 What You'll Do

• Provide primary and preventive care to aging patients
• Conduct Medicare Annual Wellness Visits and health risk assessments
• Lead Advance Care Planning discussions with patients and families
• Contribute to design and implementation of geriatric clinical programs

📋 Requirements

• Active Nurse Practitioner or Physician Assistant license in state of OK
• National NP certification (AANP or ANCC) or PA certification
• Experience in geriatric care required
• Demonstrated experience conducting AWVs and ACP discussions

✨ Nice to Have

• Experience in FQHCs or community health settings
• Prior experience building or scaling clinical programs
• Fluent Spanish speaker preferred
