💼 About This Role

You'll coordinate hospice care for patients and families in their homes, ensuring compassionate end-of-life support. You will serve as the primary nurse and case manager, driving personalized care plans with an interdisciplinary team.

🎯 What You'll Do

• Conduct initial and ongoing comprehensive patient assessments
• Develop and revise individualized care plans with interdisciplinary team
• Administer medications and treatments per physician orders
• Communicate with physicians and coordinate community resources

📋 Requirements

• Graduate of an accredited school of nursing
• Current RN licensure in the state and CPR certification
• One to two years of recent acute care or home care experience
• Valid driver's license with insured automobile
