Description

As a Home Health Outpatient Physical Therapist, you will provide outpatient services to seniors in assisted living and independent living communities, helping them optimize function and independence through evidence-based interventions. You will be part of a multi-disciplinary team dedicated to allowing clients to age in place.

Requirements

• One year prior clinical experience as a licensed Physical Therapist
• Licensed Physical Therapist (PT)
• Experience in home health or senior living preferred

Responsibilities

• Perform diagnostic tests and measurements (mobility, range of joint motion, transfer status, balance, etc.)
• Develop and implement individualized care plans including therapeutic exercises and gait training
• Continually assess and revise the care plan, participating in multidisciplinary care planning
• Educate clients and family members on rehabilitative care and independent living activities
• Accurately document client care observations and interventions using a touch pad tablet
