Description

As a home care LPN with BAYADA, you will provide skilled nursing and rehabilitative care to adult clients in their homes, helping them remain independent while you collaborate with a multi-disciplinary team. You'll perform duties like medication administration, wound care, and client assessments, documenting your work using electronic medical records on a tablet.

Requirements

• Current LPN license in Delaware
• Minimum one year recent verifiable LPN experience
• Graduation from an accredited nursing program
• Ability to work independently and manage time effectively
• Strong interpersonal and basic computer skills

Responsibilities

• Make home visits to clients in designated territories
• Administer medication, wound care, treatments, and procedures
• Monitor client conditions and report changes to management
• Perform client assessments and case management coordination
• Document care using electronic medical records on a tablet
