Description

In this part-time role, you will lead and support a team of caregivers in our Assistive Care State Programs practice, ensuring high-quality, compliant care for clients in Essex County and surrounding areas. You'll oversee clinical oversight, EVV-driven documentation, and partnership with families and payors to help clients safely remain in their homes.

Requirements

• Active RN license in New Jersey
• Experience in home health care or state-funded programs
• Knowledge of EVV systems and documentation
• Strong leadership, communication, and organizational skills
• Ability to work part-time hours

Responsibilities

• Lead, mentor, and support a team of HHAs, CHHAs, and CNAs
• Complete admission assessments, care plans, and ongoing clinical review
• Ensure compliance with policies, state regulations, and EVV requirements
• Conduct supervisory visits, home assessments, and skills validations
• Partner with families, case managers, and state agencies
