💼 About This Role

You'll lead care management for a hybrid role serving clients with complex needs in Imperial County. You'll coordinate medical, behavioral, and social services to ensure cohesive, person-centered care. This role offers the opportunity to make a profound difference daily.

🎯 What You'll Do

• Assess member needs across physical health, mental health, and social supports
• Oversee development of client care plans and goal setting
• Conduct outreach and engagement to link clients to ECM program
• Maintain up-to-date patient health records in EMR system

📋 Requirements

• Active California LVN license
• 2+ years experience as care manager or community health worker
• Willing to work hybrid (field and remote) Monday-Friday 8:30am-5:00pm
• Clean driving record, valid driver's license, and reliable transportation

• Working knowledge of government and community resources related to social determinants of health
• Experience with Google Workspace and MS Office
