Description

You will engage with individuals living with serious mental illness, substance use disorder, or homelessness, providing whole-person care through an interdisciplinary model. Most work is remote, with 2-3 in-person community visits per month.

Requirements

• Bilingual Spanish speaking
• Experience with Medicaid/Medicare populations
• Background in case management or social work
• Comfort with remote work and occasional travel
• Passion for mission-driven healthcare

Responsibilities

• Outreach and engage individuals with high medical needs
• Deliver whole-person clinical, behavioral, and social care
• Coordinate with multidisciplinary team (nurses, social workers, etc.)
• Conduct in-person community visits 2-3 times per month
• Build trust and relationships with underserved populations
