Description

You will provide skilled nursing care to home health patients in the greater Lebanon, Ohio area, following physician orders and Medicare guidelines. This role is part of a faith-based ministry dedicated to high-quality senior care.

Requirements

• Graduate of approved nursing program
• Valid Ohio LPN license
• CPR certification
• Minimum 2 years direct patient care in hospice or home care
• Valid driver's license, clean driving record, and auto insurance

Responsibilities

• Provide skilled professional nursing care to home health patients and families
• Manage primary caseload for skilled home health care patients
• Comply with physician orders, Medicare Conditions of Participation, and Ohio licensure laws
• Travel locally to patient homes in designated county
