Description

You will train, coach, and mentor newly hired RNs and LPNs in home health, while maintaining a small patient caseload. This role focuses on ensuring high-quality care for adult and geriatric patients in their homes.

Requirements

• Prior Home Health Care experience required
• Active Registered Nurse (RN) license
• Passion for clinical education
• Ability to work with adult and geriatric patients
• Valid driver's license and reliable transportation

Responsibilities

• Precept newly hired RNs and LPNs
• Provide one-on-one clinical training during home visits
• Collaborate with RN Clinical Manager and Clinical Educator on staff competencies
• Perform RN Case Manager duties including medication administration, wound care, treatments
• Monitor clients' conditions and report changes
