Description

You will be an integral member of a multi-disciplinary health care team providing palliative care to clients, enabling them to receive medical care at home. Your duties include making home visits, administering medications, wound care, treatments, comfort care, monitoring conditions, and documenting doctors' orders.

Requirements

• Current RN license in VT and NH
• Minimum one year recent clinical (medical/surgical) experience
• Prior home care or hospice experience strongly preferred but not required
• Solid computer skills; prior experience with EMR preferred
• Ability to travel to cases

Responsibilities

• Making home visits to clients in designated geographic territories
• Administering medication, wound care, treatments, procedures, and comfort care
• Monitoring clients' conditions and reporting changes to Clinical or Client Services Manager
• Executing and properly documenting doctors' orders
• Performing client assessments as necessary
