💼 About This Role

You'll provide comprehensive medical care to seniors in a PACE program, collaborating with an interdisciplinary team to develop care plans. This role offers a smaller patient caseload for true value-based care.

🎯 What You'll Do

• Perform comprehensive in-person assessments (initial, semi-annual, unscheduled).
• Develop participant care plans with IDT and ensure care coordination.
• Oversee discharge planning from acute or long-term settings.
• Make regular visits to skilled nursing facilities as needed.

📋 Requirements

• Graduation from an accredited school of nursing.
• Certification as Nurse Practitioner or Physician Assistant with prescriptive authority.
• RN state license.
• Minimum 2 years experience with frail or elderly populations.

• Current knowledge of chronic care and geriatric issues.
