Description

You will complete MDS assessments, enter data into patient information systems, and collaborate with the interdisciplinary team to ensure accurate care plans and compliance with CMS regulations. This role focuses on maintaining assessment accuracy and enhancing resident care.

Requirements

• Graduate of accredited nursing program
• Active RN license in state of work
• Two years experience with long-term care plans and MDS regulations preferred
• Excellent communication skills for interacting with residents, visitors, and staff
• Ability to manage time and organize daily schedule to meet productivity standards

Responsibilities

• Complete all MDS assessment forms per RAI manual guidelines
• Enter MDS data and transmit to CMS within required timelines
• Ensure all RAI participants complete their sections in regulatory timeframes
• Communicate with physicians and team to maintain accurate assessments
• Develop annual goals aligned with team objectives
