💼 About This Role

You'll provide home health visits to adult and geriatric patients as part of a multi-disciplinary team. Your core impact is enabling clients to remain safely at home through rehabilitative care.

🎯 What You'll Do

• Make home visits to clients in designated geographic territories.
• Perform evaluations for speech, language, communication, or swallowing difficulties.
• Develop treatment plans based on evaluations and physician orders.
• Educate patients and families on rehabilitative care activities.
• Document observations and interventions using a touch pad tablet.

📋 Requirements

• Current SLP license in Colorado.
• Certificate of clinical competence from ASHA or equivalent.
• At least one year of SLP experience.

• Prior home health experience as an SLP.
• Experience with electronic medical records (EMR).
