Description

As an MDS Coordinator at San Francisco Campus for Jewish Living, you will ensure timely and accurate completion of MDS assessments, ensuring compliance with Medicare, OBRA, and Title 22 guidelines. You'll manage the care planning process, conduct chart audits, and coordinate care plan conferences to support quality care for older adults.

Requirements

• Experience with MDS assessments and care planning
• Knowledge of Medicare, OBRA, and Title 22 regulations
• Ability to coordinate interdisciplinary team
• Strong organizational and communication skills

Responsibilities

• Assure facility compliance with Medicare, OBRA, and Title 22 guidelines
• Support identification of Medicare entitlement and eligibility for prospective residents
• Perform utilization review (UR)
• Oversee gathering of information for assessments and care plans
• Schedule and facilitate care plan conferences
• Participate in committees to address and resolve issues
• Input or supervise input of MDS assessments and transmission to state
• Manage care planning process
• Complete chart audits to assess documentation support for MDS assessments
• Identify residents with significant change in status and manage interdisciplinary process
• Monitor MDS documentation and charting requirements to support billing
• Assure timely completion of required reports and communication
