🎯 What You'll Do

• Assess healthcare needs and identify appropriate interventions.
• Develop and maintain individualized care plans with regular follow-ups.
• Conduct proactive outreach to members, providing education and coordination.
• Monitor and improve clinical, financial, and functional outcomes.

📋 Requirements

• Current RN licensure in PA, multi-state eNLC, or active license in WV/DE/NY.
• 7+ years combined experience in clinical care, case management, or health insurance.
• Strong understanding of disease processes and care coordination practices.
• Proficiency in Microsoft Excel and data interpretation skills.

✨ Nice to Have

• Bachelor's degree in Nursing and Case Management certification.
• Experience in motivational interviewing, CBT, or DBT.
