💼 About This Role

You'll join an interdisciplinary team (IDT) to provide direct care and psychosocial support to participants. You'll manage a caseload, coordinate community services, and help participants achieve their full potential. This role offers growth in a supportive environment with a focus on patient-centered care.

🎯 What You'll Do

• Establish rapport with participants and families for psychosocial support
• Deliver and document social work interventions within scope of practice
• Coordinate care services such as housing, DMV needs, utility bills
• Complete IDT care plan reviews with participants and family members

📋 Requirements

• High school diploma or equivalency
• Associate degree in social work, human services, or relevant field preferred
• Reliable means of transportation
• Obtain CPR/First Aid certification within 180 days of hire

✨ Nice to Have

• Previous experience with behavioral health and substance abuse issues
• Previous experience facilitating solutions with difficult psychosocial circumstances
• CPR and First Aid certification upon hire
