Description

You will provide rehabilitative speech therapy to adult and geriatric patients in their homes, conducting evaluations, developing treatment plans, and educating patients and families to support independent living.

Requirements

• Current Speech Language Pathologist license in North Carolina
• Certificate of Clinical Competence from ASHA or equivalent educational/work experience
• Home health experience required
• Solid computer skills; EMR experience preferred
• Available 2-3 days per week

Responsibilities

• Make home visits to clients in designated territories
• Perform evaluations for speech, language, communication, or swallowing difficulties
• Develop and revise treatment plans based on evaluations and physician orders
• Educate patients and families on rehabilitative care and activities
• Document observations and interventions using electronic medical records
