You will deliver in-home physical therapy services to adult and geriatric patients in and around Tyler, helping them restore strength, balance, and mobility to safely navigate their homes and communities. Enjoy flexible scheduling, consistent patient volume, and supportive team environment.

Requirements

• Active physical therapy (PT) license in good standing
• BLS CPR certification
• Reliable transportation for home visits
• Basic comfort with technology (email, texting, EMR systems)
• Training provided for home health transition

Responsibilities

• Deliver in-home physical therapy to adult and geriatric patients
• Evaluate mobility, strength, and function to develop treatment goals
• Implement rehabilitation and therapeutic exercise programs
• Educate patients and families on recovery plans and preventive care
• Maintain accurate home health care documentation per Medicare standards
