You'll provide hands-on patient care in the home health setting, working under a Case Manager RN to implement plans of care and educate patients and families. 🎯 What You'll Do

• Provide hands-on patient care including wound care, catheter management, and IV therapy.
• Implement and document care plans within 24-48 hours under Case Manager direction.
• Educate patients and families on diagnosis, treatment, and self-care techniques.
• Coordinate with interdisciplinary team and attend weekly case conferences.

📋 Requirements

• 1 year of recent nursing experience (may be waived for new grads).
• Current CA Registered Nurse License.
• Current BLS certification.
• Valid CA driver's license with acceptable driving record.
