As a PRN Hospice Aide at Luminary Hospice, you will provide personal care and comfort to patients at the end of life, working under the supervision of a registered nurse. Your role focuses on observing and documenting patient needs, reporting to the care team, and ensuring compassionate support for patients and their families.

Responsibilities

• Perform personal care needs after patient assignment
• Attend to patient requests promptly
• Report observations and care performed to Case Manager
• Document observations and care performed
• Promote quality, continuity, and safety of patient care
