Description

You will manage the administrative functions related to onboarding, credentialing, and ongoing maintenance of Hazel Health's provider network. You'll coordinate end-to-end onboarding, track documentation, update provider records, and ensure compliance with regulatory standards to support high-quality patient care.

Requirements

• Manage provider onboarding activation and credentialing processes
• Create and maintain provider records and CAQH profiles
• Collaborate with internal teams and external vendors
• Ensure compliance with regulatory standards
• Work remotely in one of the listed states

Responsibilities

• Coordinate end-to-end onboarding for new providers, ensuring timely setup across platforms and systems
• Collect, track, and follow up on outstanding documentation to maintain compliance and onboarding timelines
• Enter, update, and maintain provider records in Salesforce and CertifyOS
• Activate providers in alignment with confirmed start dates for operational readiness
• Manage and monitor credentialing applications to ensure completion prior to start dates
