Care Support Coordinator (Temporary)

Description

You will serve as the primary point of contact for members and providers, coordinating care delivery through inbound and outbound communication channels including calls, email, and chat. Your role involves de-escalating situations, managing administrative tasks like appointments and documentation, troubleshooting technical issues, and advocating for clients to ensure high satisfaction.

Requirements

  • Ability to work any of the listed shifts (8am-4pm, 9am-5pm, 10am-6pm, 12pm-8pm, or 3pm-11pm EST)
  • Availability to work 1 weekend per month and at least 4 holidays per year
  • Agility and flexibility to support fluctuating volume of work
  • Effective communication and active listening skills
  • Empathy and ability to handle sensitive information

Responsibilities

  • Support inbound telephonic communication with members and providers (approx. 7 hours daily)
  • De-escalate situations and resolve concerns using solution-focused approach
  • Manage care coordination: maintain patient charts, reschedule appointments, execute prescription authorization forms, facilitate documentation release
  • Analyze and troubleshoot technical issues submitted by members and providers
  • Collaborate cross-functionally to streamline processes and improve service experiences
