Description

As an LVN with Remedy Home Health Care in Maricopa County, you will provide hands-on skilled nursing care to patients in their homes under RN supervision. You'll follow individualized care plans, document visits using EHR, and communicate with patients, families, and the care team to help patients regain independence and improve quality of life.

Requirements

• Current Arizona LVN license in good standing
• Graduate of accredited vocational nursing program
• Minimum 1 year nursing experience (home health preferred)
• Physical ability to assist and move patients
• Proficiency with electronic charting and care coordination software

Responsibilities

• Provide skilled, patient-centered nursing care based on individualized care plans
• Accurately document visits, assessments, and treatments using electronic health records
• Communicate effectively with patients, families, and the home health care team
• Ensure compliance with Arizona home health regulations and company policies
