Description

You will provide medical social work services to clients in their homes as part of a multidisciplinary team. Your role includes evaluating social and emotional factors, connecting clients with community resources, and advocating for their needs to help them remain at home.

Requirements

• Master's in Social Work with 2+ years community-based experience
• Accreditation by Council on Social Work Education
• Effective communication in English
• Ability to travel to cases
• Solid computer skills; EMR experience preferred

Responsibilities

• Conduct home visits and clinical social evaluations
• Plan interventions based on evaluation findings
• Identify and refer to community resources
• Advocate for clients and families navigating the healthcare system
• Collaborate with multidisciplinary team on care plan development
