You'll serve as the primary point of contact for clients with complex needs, coordinating care across providers. You'll develop care plans, advocate for clients, and connect them to community resources in Imperial County. This hybrid role offers the chance to transform care for vulnerable populations with a supportive team.

🎯 What You'll Do

• Assess member needs across physical and mental health domains
• Develop and oversee client care plans and goal setting
• Conduct outreach and engagement for ECM program linkage
• Complete documentation and maintain patient health records

📋 Requirements

• Active ACSW, LCSW, LMFT, or LPCC license in California
• 1+ year experience as care manager or community health worker
• Valid driver's license and reliable transportation
• Working knowledge of government and community resources
