Description

Join One Medical Seniors in Sun City, AZ to provide comprehensive primary care in an outpatient, value-based setting. You'll partner with a dedicated care team, leverage coaching and behavioral health models to engage geriatric patients, and utilize our proprietary EHR to plan care and drive population health outcomes.

Requirements

• At least 2 years of primary care experience
• Current Arizona medical license or ability to obtain
• Board Certification in Internal or Family Medicine
• Ability to build relationships and communicate effectively

Responsibilities

• Provide best in class comprehensive primary care in an outpatient setting
• Partner with care team and leverage coaching and integrated behavioral health models to engage geriatric patients
• Utilize proprietary Electronic Health Record to plan care and maximize population impact
• Participate in weekly protected time for team building and case discussions
