💼 About This Role

You'll perform home health visits for adult and geriatric patients in the Charlotte area as part of a multi-disciplinary care team. Your work enables clients to remain safely at home while recovering or managing conditions.

🎯 What You'll Do

• Conduct home visits for occupational therapy evaluations.
• Develop individualized intervention plans to improve functional status.
• Assess home environment for hazards and equipment needs.
• Document patient care using a touch pad tablet and EMR.

📋 Requirements

• Current North Carolina Occupational Therapist license.
• Minimum one year of recent work experience as an OT.
• Graduation from an accredited occupational therapy program.
