💼 About This Role

You'll provide skilled home health services to help patients live safely and productively in their homes. You'll serve as an educator and patient advocate as part of an interdisciplinary care team. This role offers flexible schedules and targeted territories.

🎯 What You'll Do

• Evaluate patient needs for home safety and medication management
• Provide wheelchair seating and positioning interventions
• Develop pain management and falls prevention plans
• Educate patients and families on chronic disease and dementia care

📋 Requirements

• Current unrestricted CA Occupational Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
