Description

You will conduct in-home assessments and functional evaluations for older adults and high-needs populations, delivering personalized home modification recommendations. You'll collaborate with clients and caregivers on fall prevention and care planning, while maintaining documentation through an electronic system.

Requirements

• Degree in Occupational Therapy from accredited program
• Active unencumbered OT license
• 2+ years clinical experience
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation
