💼 About This Role

You'll serve as the central point of contact for our organization on weekends, managing Kaiser referral intake and supporting field clinicians with clinical escalations. You'll ensure patients receive timely, seamless care coordination when weekday leadership is offline. This role offers autonomous work in a fast-paced environment at a Top Workplace home health agency.

🎯 What You'll Do

• Manage Kaiser referral intake and coordination on weekends
• Assist field clinicians with urgent patient reassignments and schedule gaps
• Triage and respond to clinical escalation calls from staff and patients

📋 Requirements

• Active California LVN or RN license in good standing
• Minimum 2 years of clinical experience
• Valid California driver's license with acceptable driving record

✨ Nice to Have

• Home health experience strongly preferred
• Prior intake, scheduling, or care coordination experience
• Proficiency with CareStitch, Kinser, or similar home health platforms
